Licensed Practical Nurses Positions:

LPN’s- Part Time Evening Shift 3:00 pm-11:15 pm

LPN- Part Time  Day Shift- 7:00 am- 3:15 pm

QUALIFICATIONS: Licensed in the State of New York as a Licensed Practical Nurse. Graduate of an accredited school of practical nursing. Two years of clinical experience in a long-term care setting preferred. Must also possess and maintain all necessary certifications for CPR/BLS, and other required Nursing in-services as required by this Facility.

GENERAL OBJECTIVES:  Performs tasks and responsibilities within established guidelines, under the direction of a Registered Professional Nurse. Delivers care that maintains the dignity and self-worth of the individual that is consistent with the philosophy of the Nursing Department. Demonstrates the ability to work effectively with others.

Certified Nursing Assistants:  

Must possess current NYS certification. Prior experience in similar setting preferred.  Will train right candidate.

Positions:  Per Diem- All Shifts.

QUALIFICATIONS:  Certified as a Nursing Assistant in New York State.  A High School graduate or equivalency. H.S. Diploma preferred. Previous experience as a Certified Nursing Assistant in a Long Term Residential Care Facility strongly preferred.   Must submit to Criminal History Records Check (CHRC) as per federal regulations.

GENERAL OBJECTIVES:  The Certified Nursing Assistant provides quality care to residents in compliance philosophies, objectives and established policies and procedures of the facility.  The Certified Nursing Assistant assists residents in a maximum potential for self-care and level of independence.  They ensure that residents/patients are comfortable, safe and maintain good hygiene and personal cleanliness.


Position: FT Days/Eve/Weekend Flexible


Must have successfully completed, or be in the process of completing, C.T.R.S. certification, MT-BC Music Therapist certification, ART-BC Art Therapist or other certification. Have thorough knowledge of various resident activities, and have a familiarity with resident limitations and needs; be able to work in close cooperation with the Director of Therapeutic Recreation, as well as display initiative to complete activities, related tasks and programs. Must have appropriate experience in a long-term care setting.


The Recreation Therapist will assist in the planning, coordinating and implementation of therapeutic activity programs on a daily basis to meet the individual resident’s needs and interests.  The Recreation Therapist will be responsible for filling out all facility documentation in regard to Therapeutic Recreation following State and Federal guidelines.
